  • live in the Borough of Barnet
  • be aged between 4-18 years
Innovation Dance School
  • Attend the dance school in Barnet or Ealing
We offer up to 12 weeks of therapy.

To self-refer to either of our CAMHS funded services please complete the form below and tick which service you require.

Once submitted you will be contacted and provided with further information on how the service runs and when an appointment will be available for you.

For Professionals:- Please provide your name as the referrer and also the client/parent/carer contact details for appointments to be arranged directly.

Once you have completed and submitted a form you will be sent an email acknowledging receipt of your information. In due course you will receive a telephone call to further assess your needs and the process of being placed with a therapist will be explained to you. Due to the high demand for our services there is a waiting time of approximately 12 weeks.

NB. Professionals or self-referral are accepted however professionals need to provide the contact details of the person/parent/carer of the person they are referring please.
